To recreate the map, I created tight, single-player wide corridors throughout on a single floor elevation.  No ammo or health packs were placed inside to establish the suicidal condition of the players.  Above is a screenshot of the paper design with the addition of the new game mechanic, a compression chamber.  In this room, when the doors on either end close, anyone trapped inside is meet with a deadly surprise.
